Registration is now open for the 2010 Michigan Primary Care Association (MPCA) Annual Conference, “Home Sweet Patient Centered Medical Home.” The agenda is rich with resources, best practices, lessons learned, and success stories focused on the nationally recognized Patient Centered Medical Home principles.

The MPCA Annual Conference will provide Health Center staff and board members with information about resources designed to assist them in transitioning their Health Centers into Patient Centered Medical Homes that allow community members to participate in comprehensive, coordinated, continuous health care that is centered on their needs needs and integrates care across the entire community and health care system.
